Before the patient’s nurse practitioner returned home for the day, she made a special visit to the pharmacy, ensuring that the patient's medications were taken care of and covered by our ministry team. What's more, she picked up dinner for our patient, who was facing a food shortage that evening. At Segue, we believe in the importance of not only addressing your medical needs but also providing genuine care and support during your journey to recovery.

If I already have home health, why do I need Segue's transitional care?

First, please know that Segue is not a replacement of your home health service. We work with them to make sure that your first 30 days are as trouble free as possible. Our nurse practitioner can prescribe medications if needed and diagnose and treat any new conditions.

Will I have to pay out of pocket for this care?

We bill your insurance company for our services, and you will receive a statement in the mail for any unpaid balance. As a non-profit organization, any payment received from you will go directly to helping others who are less fortunate. If you do not have the ability to pay, no payment will be required.

What if I need you after you've already been to my home?

We are available 24 hours a day, 7 daysa week. Just pick up the phone and giveus a call. Even if it’s not health related, our 24-hour ministry team is available to assist in any way we can.

We are a small team who cares about you personally — body, mind and soul. We recognize that physical needs have spiritual consequences. That’s why we want to be a resource to you and your caregivers by helping in a loving, Christ-like and Biblical manner with issues related to your spiritual health.
